Why do you need CareLink?
As people grow older, concerns for day-to-day living and health care increase. Families face difficult decisions, and many challenges. The responsibility of caring for these elders frequently rests with the children or family members who need to make important decisions. Often the closest family members are not geographically nearby. Sometimes family members are overwhelmed and need help determining what the best course of care is for their loved ones.
When you call CareLink, you speak to a trained eldercare professional who will gather information about your concerns regarding your family member or loved one. A Care Manager will visit your loved one for an in-home assessment of his/her situation. This assessment focuses on their strengths and coping skills, as well as their ability to care for themselves physically, socially and psychologically. Our assessment explores how CareLink can add to their quality of life through referrals to community services, supportive counseling, and the knowledge that there is someone nearby on whom they may call at any time.
